User Task form field not visible in Tasks UI – only “Done” button visible

I’m working on a Collibra workflow where I’ve added a User Task with a form (basic text fields) attached to it.
What I’ve done:
Created a User Task
Created a form and added fields (text inputs)
Referenced the form in the User Task
Saved and deployed the workflow
Started a new workflow instance
Issue:
When I open the task from Tasks, the UI shows:
Task title
Due date
Only a Done button
The form itself does not render at all (blank content area).
Is there a specific configuration required to “publish” or activate a form?
